By registering as a member of JMU Lifelong Learning Institute (LLI), you consent to the internal, non-public disclosure of
your address, telephone number and email addresses to faculty and staff of JMU and the LLI including official committee chairs of the LLI Advisory
Board for the purposes of administering the LLI program. This includes facilitating class communications. You provide this consent with the knowledge
that JMU, including LLI, will not further disclose your contact information to anyone outside of JMU, or LLI, without your written consent.

Media Release: I hereby agree to give JAMES MADISON UNIVERSITY, their assigns and legal representatives the irrevocable right to use photograph or
video images of me in all forms and media for education or other lawful purposes in its publications and displays. I waive my right to preview or approve the finished product, including written copy that may be created in connection therewith.

Acknowledgment of Understanding
Code of Conduct
All JMU LLI members are expected to behave in a manner conducive to a successful and enjoyable learning experience. Sometimes, relatively harmless things can disrupt the study group dynamic; for example, some members unintentionally dominate discussions. Members must think about their role in class as individuals but also as part of the group experience. Some disruptions are of a more serious nature and cannot be tolerated. These policies extend to student interaction with fellow members, instructors, and JMU staff.
As members of the James Madison University community, LLI members are expected to behave with each other with civility, kindness, and respect, recognizing that disagreement and informed debate are valued in an academic community. Demeaning, disrespectful, offensive, intimidating, threatening, violent, or otherwise hurtful or unsafe behaviors will not be tolerated. Such behavior could result in dismissal from the program at the sole discretion of the LLI Director or SPCE Leadership.
Assumption of Risk
I understand that although JMU LLI holds the health and safety of its community as paramount, this does not guarantee that I will not be exposed to injury or illness of any kind.
I acknowledge that my physical presence in the local community as well as participation and utilization of facilities, services, and programs at JMU LLI is voluntary, and may carry certain risks, such as injury or illness that cannot be eliminated regardless of the care and reasonable efforts taken to avoid and mitigate those risks.
I also understand that I may be at higher risk for injury or illness and take full responsibility for my health and safety.
I understand that I have the option to participate in JMU LLI activities remotely. Despite these risks, I chose to participate in-person in JMU LLI activities.
I agree not to attend any in-person sessions if I am experiencing any symptom of illness such as cough, shortness of breath or difficulty breathing, fever, chills, repeated shaking with chills, muscle pain, headache, or sore throat.
I hereby release and agree to hold JMU LLI harmless from, and waive on behalf of myself, my heirs, and any personal representatives any and all causes of action, claims, demands, damages, costs, expenses and compensation for damage or loss to myself and/or property that may be caused by any act, or failure to act of JMU LLI, or that may otherwise arise in any way in connection with any services received from JMU LLI.
I understand that this release discharges JMU LLI from any liability or claim that I, my heirs, or any personal representatives may have against JMU LLI with respect to any bodily injury, illness, death, medical treatment, or property damage that may arise from, or in connection to, any services received from JMU LLI. This liability acknowledgement and release extends to JMU LLI together with all venues, volunteers, and employees.
Media Release
I hereby consent to being photographed by James Madison University (the "University") or anyone authorized by the University ("Agent"), and I hereby grant permission to the University or its Agent to use or reproduce any such video or photographs for the University's educational or promotional purposes only, in print, electronic form, or other media without any further compensation to me. Such video and photographs shall be solely owned by the University. I hereby waive any right to inspect or approve such photographs or electronic matter that may be used in conjunction with them now or in the future, whether that use is known to me or unknown, and I waive any right to royalties or other compensation arising from or related to the use of the photographs. I hereby agree to release and hold harmless the University from and against any claims, damages or liability arising from or related to the use of the photographs, including but not limited to any re-use, distortion, blurring, alteration, optical illusion or use in composite form, either intentionally or otherwise, that may occur or be produced in the production of the finished product.
